After 45 days there is nothing you can do through eBay or PayPal. If you paid by credit card and not by PP balance, then call your credit card company and request a chargeback since the seller did not ship the item for which you paid.
Being "in the hospital" is one of the most common excuses I hear from buyers for non-payment. You can never know for sure if it is legitimate but keep that in mind whenever someone uses it for buying or selling.
You should never let it go past the 45 day mark even if the seller does have a valid reason. That extends your protection for a longer period of time and you can always close the case when the article has been received.
You can give an item number on eBay boards but using personal information and user id's is against eBay policy.
