I am planning to ship from my local UPS office. When I enter my "ship from location" do I enter my personal address or the UPS offices address?

The reason is in the name "return address".

Do you want the parcel, if it must be returned to go to your house (or business) or to the UPS office?

If you have a mailbox at the UPS office, you could use that, of course.
