Do you have any rules set up for combined shipping? If no rules, then all the shipping costs are included. No idea if the cart is allowing a request for combined shipping or a revised invoice to be sent (there have been reports of it not working properly).
PayPal for another month and a bit, will still return fees on refunds made by a Canadian Seller (after May 2020 PayPal will keep all their fees).
If you know what items he wants to buy, you could set up a special listing for him of all the items with a customized ship cost. And then send the combo link to him.
