Is it possible to have multiple shipping options with calculated and fixed rates? I want to offer Canada Post with calculated shipping, and a second option with a fixed rate, but the second option always follows the first's payment structure.
I've also tried setting a generic method fixed rate as the primary shipping, but then Canada Post calculated shipping is not an option for the second shipping service. If I select Canada Post with a fixed rate, the second option also has a fixed rate.
I would like to be able to offer Canada Post calculated shipping, but up to a maximum so someone in Canada far away from me doesn't pay the upper bound of that calculated shipping. Offering a second fixed rate shipping was how I was trying to do this.
I've also tried surcharge-based rate tables, but I can't find the option to apply it to a listing.

I'm not sure. I use business policies so the options are under there for me. In a business shipping policy you would see something like:
And then in the listing you would choose that shipping policy:

I use Flat Rate shipping (usually "free" shipping) domestically.
But Internationally I use Calculated Shipping.
This means that I can set different rates for different locations (USA/ UK/Australia/etc.)
But I can't set Flat Rate for USA then Calculated for the rest of the world.
Only domestic and international can differ.
In my opinion, it's best to KISS.
Offer one shipping method only.
The one that is most prompt, secure and economical for YOU.
If the buyer has a special request, you can assess it and change the shipping method as a special favour.
Be aware that most will want cheaper, untracked, slower and less secure shipping because they are setting you up.

As @reallynicestamps said, you can't set calculated and flat rate for the same region. I've been asking for that for a few years now (or for them to add Canada Post flat rate boxes to the calculated options), but it doesn't appear likely to ever happen.
Personally, I'm using rate tables to set shipping costs for most items. It's not perfect (sometimes you make a little and sometimes you lose a little), but with a little tinkering you can get it pretty close and it gives you the flexibility to use whatever shipping company makes sense for a given parcel and destination.

My KISS solution is rate tables, based on Stallion Express courier rates for now due to to CP uncertainty (I know not everyone has access) - when CP is working cleanly, I use their CP zone skip rates.
With the spreadsheet provided by eBay in the rate tables setup section that provides postal codes broken down by rate table regions and the estimator in my stallion account page, it's about a 25 minute exercise. I add 15% to the quotes since they don't include HST/PST. I don't consider UniUni as an option since their crowd sourced last mile makes me nervous.

I had to enable the Seller Hub to get these options for the rate table.
