I recently had 5 separate items listed. The buyer purchased all 5 items, so I shipped them together. Does that only count as 1 sale on my feedback or should it be 5 sales since they were listed separately?

If the buyer paid them on a single invoice, it will count as only one sale = one feedback. They would have to be 7 days apart to count as 2 or more.

All purchases from one buyer are combined in the feedback rating and count as one when purchased the same week (Monday morning to Sunday night). The feedback numbers will show the total number of feedback left by the buyer (five in your case) but the rating will count for one. The same applied to DSRs.
Now, if a buyer purchases an item on Saturday and another one on Monday and leaves two positive feedbacks, the rating will go up by two (2) even if both purchases were paid together on the same payment.
