If the line credit cards processed by Paypal is written does that mean that you have no choice but to use Paypal for that item?

It means you can pay with a credit card without having a PayPal account. 

Sellers use paypal to process the payment unless they have their own merchant agreements with the credit card company. Some entity has to process it, that doesn't happen by magic. Even when you use your debit card to pay at a grocery store, the payment is being processed by a third-party.

And the service costs the merchant plenty.
Paypal is about average in its charges to the seller.
Ask a merchant who has accepted American Express. Tell them first that you will 'pardon their french'.
