Has anyone else received forms to fill out from their bank? Is this because of the PayPal account?

Just the PayPal account. Don't know if that would be considered an account with a US bank.

Hmm well I'm not much help. I've had to sign a whole bunch of forms for my US account from a US bank located in the USA, I think this was one of them. If you only have PAYPAL I don't know why you got one.... hopefully someone else will know....

Are you an American citizen? Are you an American resident? The intent of the Act seems to be to catch US citizens that have assets in Canadian financial institutions either because they are current residents of Canada or do business here.
