The easiest way is not to give the buyer options.
Tell the buyer what service you will use.
Since Delivery Confirmation and Insurance benefit you and not the buyer, he has no reason to opt for them.
If you offer a fast and expensive shipping service, your buyer will choose the cheaper one and expect the fast one. It's human nature.
And you get negative feedback in your stocking.
If your items are "gifty" it would be a good idea to offer only the fastest methods of shipping. (Well actually, it would be a good idea not to be listing anything with only five days left for delivery)
