I sent two message to let him know that the item was ready to get pick up at his local post office and he received a card to where his suupposed to pick it up. But I didn't get any answer from the buyer. What can I do next? If the item came back to me. Do I refund that person and relist my item?

Both you and the PO have told your buyer about his next step.
Your basic responsibility is over.
If the item comes back to you, and it could be a couple of months, refund and relist. Personally, i would only refund the selling price and not the cost of shipping. If the PO charged me return shipping, I would also reduce the refund by the cost of the return shipping.
