Do I simply go to Paypal and "Issue Refund" there? Will I get a refund for my eBay fees this way?

Are you refunding without return of the item.
To refund in PayPal.. click on "details" of the buyer's payment and scroll down to refund link. PP will credit most of your receiving fee.
After refunding and on good terms with the buyer , ask if they would accept a mutual cancellation so you can recover some of cost paid to eBay

Where do I go for the Mutual Cancellation? that will do for us, if I understand it all right. He bought the item, paid, received it and is not satisfied. I want to refund my customer. So is still "Mutual Cancellation" ok? Or is it only when item has been paid but not shipped and received? Si if it,s good, again, where do I find this "Mutual Cancellation" link and process? thanks again
The easiest way to do the refund is, as you said, through Paypal.
This will not get your fees refunded though, for that you have to cancel the transaction.
There are two ways to do this.
If you and the customer are friendly and polite, ask her to agree to a Mutual Cancellation. You can do this immediately, she can agree immediately, you get your fees and she can leave feedback.
That last point is the problem. If she is at all belligerent or angry, go for the Unpaid Item Dispute. This cannot be opened for 96 hours, nor closed for another 96 hours. But if she does not pay, you get your fees and she cannot leave feedback.
