I have a buyer who is being nit picky with the condition of a 70 year old item the I believe that the condition was well described in the description.
He has already left positive feedback.
Can a dispute be opened if the buyer has already left positive feedback?

Yes.
But you might be better, after putting him on your Blocked Buyer List, to remind him that if he returns the item he gets a full refund of his original payment.
But you know your business better than I do.

Thanks - what I really think he/she is angling for is a partial refund. I checked his feedback left for others and there is some indication that he may have done this type of thing before
