Is there any easy way to do this on items already "Buy It Now'd" in US dollars?

Send them a revised invoice in Cdn. Dollars. That's what I have done, but I don't know if the Buy It Now makes a difference.

Send an invoice in Canadian dollars through Paypal.

This has always been my choice

How do you revise a combined invoice out of selling manager from US into CDN?

It cannot be done.  eBay can only invoice in the currency of the listing. 

If invoicing in Cdn$ is required, send an invoice through PayPal and make manually adjustment once payment has been received.
