Personally, I always use the zip code following USA on the last line.  For example:

(name of addressee)

URB Algarrobo k18 calle G
Guayama, PR 
USA                            00784

In this case, State (or territory) should read PR (not P.R). All US States or Territories have a two upper case letter abbreviation.

It actually has a choice for Puerto Rico even thoufg it's treated like any US State.

Puerto Rico is odd. They have a variety of address formats. I like to use exactly what they use as they mean different things in different parts of the Commonwealth of Puerto Rico.
