CBSA will use tape and forms to make it clear that the item was damaged while in their possession. Is this what happened?

If the damage happened in Kentucky at the PB plant of the Global Shipping Program, there will be a form with the eBay logo . Is this what happened?

There was a form. I'll try to send a picture via email. it looks as if the customs person cut through the packing with an exacto knife so hard that he cut into the equipment I purchased.
