I have a couple of items I have sold to U.S. buyers. I picked up some small packet customs forms from Canada Post. It is not a gift, or sample, or documents, so I am assuming I check off the "other documentation" box. What do I write down beside the checked off box though? Is "ebay merchandise" OK?

Hello 'jessica',
It's merchandise. The 'where' it was bought is irrelevant, - only what it is, "Eyeshadow" for example. You will want to say what it is because some items of merchandise are non-mailable. Yours are fine.
And the declared value of any item is always the amount paid for the item, - not more not less. It's what you can prove on paper.
