Tell him politely that you are sorry that he is disappointed, and that he should return the item for a full refund.
When he does, refund him.
If he files a dispute, he will be told that he should return the item for a full refund. He will be required to supply Confirmation of Delivery or he will not get a refund.
If he does return the item, refund promptly and tell Paypal that you did.
If you don't refund, PP will and then will come after you for their money. In addition, eBay will put a black mark on your account.
No returns does not mean no refunds. Surprise! If you take PP you make refunds. So you may as well get your item back too.
When you refund, you will get your FVF on price and shipping back.
If he refuses to return the item, you do not have to refund.

By the way, manufacturer's sizes are useless.
I wear at least four different sizes on any given day, because the measurements vary from maker to maker. And the variation is by price point.
DD recently indulged in a pair of Lululemon yoga pants. Normally she shops at WalMart and wears Size Ten. The Lulus were Size TWO!!
So, to prevent problems in future, add measurements to your listings.
Bust, waist , hip.
Length from shoulder to waist or to hem.
Length of pant leg (" Ankle length" is meaningless. DD is 4'11". Her best friend, who often borrows her clothes, is 6' tall. They wear the same size, just differently.)
Length of sleeve. And in plus sizes, width of sleeve at bicep.
