My question is about charging for this extra fee for local pick up ($5).  Once the item is sold and someone agrees to it, how do I add it to the final sale price?

All pick up transactions should be cash only. If you accept PayPal , always the possibility of a claim with no way to prove delivery.

Meet , exchange , mark sale as paid , leave feedback..end of story.
