I sold an item 3 months ago to the buyer. He contacted me today saying that he isn't satisfied and wants a refund.
The item worked fine when I sold it but he is only telling me now - 3 months later, that it hasn't been working properly since the start.
I am guessing he might have damaged it himself but I have no way of knowing.
What can I do in this case? I don't want to be reported on the site or have to pay the massive $170 to get it sent back to me, only to be -$340 after attempting to sell it.
Also, I did mention no refunds on the listing.

Just tell him it is too late to file a claim.
It is too late for him to file a claim with ebay or paypal.
Had to be filed within 45 days.
However if he happened to pay with a Credit Card through
paypal there is a chance he can do a chargeback as credit card companies
have different times for filing a claim.
I would also put him on your blocked bidders list now...

I believe credit card companies give 90 days from payment date to file a chargeback so the buyer may have exceeded the chargeback deadline as well.

Normal practice is that the unhappy buyer has to return the item, with Proof of Delivery and sometimes Signature Confirmation, to the seller on his own dime, before the seller is required to refund.
Most credit cards will not support a refund claim like this after 90 days. This is twice as long as Paypal/eBay.
FWIW, he can't leave feedback now either. He can leave a Response to existing feedback. So can you.
Tough it out. If you want to be really nice, you can tell him to return the item for a refund of the selling price (no postage either way). But after all this time, I wouldn't.
Usually, these demands fall apart once the bully realizes you won't cave, and that he will have to pay postage and return the widget to get anything back.
