Canadian seller shipping from USA

Does anyone have current info on Canadian crossing the border with packages to ship in USA?

Years ago I did this and we had to fill out a form and pay a service fee to do this. Is that still the case?

@recped  posted the link to a post I did this week with the form info, etc.   

It is frustrating that you can search the web and read through tons of CBP info and not find the info you need for this situation.  As posted, I did my best but still had the wrong form.
