I have a buyer in the UK saying they did not authorize a payment for a pattern. I don't understand, does this mean that anyone can purchase an item and then claim they didn't authorize it, and get the payment refunded and keep the item?

You should contact PayPal to see what's going on. Because it's going to depend on whether it's a credit card chargeback or something else.
Is your item tracked? Because tracking will be of use if it's to an address associated with a credit card. Harder to make a not authorized credit card purchase claim stick if the purchase is shipped to the credit card address.

Hi Triber, is this a chargeback or did they just let you know they didn't order the item?

No, it was letter mail, so tracking would be way to high. Nothing can be done, I have no documentation that I even mailed it. It just seems unfair that buyers do things like that. I don't feel I want to stop shipping Internationally because of one bad buyer, but it does make you feel vulnerable. I will call Paypal, but without tracking I don't think there is any recourse.

Aaaaand - once again I murble on about Cookie Jar Insurance.
Most people are honest, but there are a few, and there are honest differences of opinion.
Cookie Jar Insurance is just adding a few pennies to every asking price and putting those virtual pennies in a virtual cookie jar against problem transactions.
When a refund is needed, the seller pays from the Cookie Jar, without questioning the buyer in any depth. Again, most people are honest about a problem.
And time is money.
About 18 cents a minute here in BC at minimum wage.I guess that will rise now that we have an NDP government.
The seller has to put her feelings aside and ask herself, how much did this item cost me? How much will it cost to fight the refund?
And do the arithmetic.
It's not personal. It's business.
