How do I add shipping rate for several Canadian provinces (Domestic) and USA (international)?

I want to specify a shipping rate for several Canadian provinces (Ie: Toronto, Halifax, St. John's, etc.), and also the US...how can I achieve this?

Anyway, use Calculated Shipping and optional metric measurements. (Canada Post went metric in 1974, but eBay hasn't figured that one out yet.)
