Insurance process on Small packages....?

Hi there,
I shipped 2 small packages on Jan 12 (the date that the insurance fiasco started on the packages leaving Canada to the world, 100$ that was included on all packages but with no tracking #'s)
I shipped a package to USA that has not arrived yet (mind you it is ground mail but still should have arrived by now) and I shipped one to UK wich only takes 3-5 days as I live on the EAST coast (I ship to the same customer all the time) and it has not arrived yet. There were tons of air mail stickers on the package and I paid full price (6.40$).
Now the fun part. after 30 days, who do I call? What application do I fill out? what do you tell them? the address and name of the client?
thanks for any help I can get on these issues.

As far as i know you just need to have your receipt handy (i wonder what they do if you just put stamps on the envelope and have no receipt??) and call canada post customer service...they assume that you are honest, and will start claims (although there is no way to prove if the item was ever delivered!)

Thanks for the info.
I have kept all my receipts and the fun part is taht I usually ship stuff in Canada and US through EXpedited so it always puts my Account # on top of each receipt.

Will be calling them on FEB 13
