I was approved for a refund on a item that I bought, because the seller never sent it (loads of other people were having the same issue with this seller). The day following I get charged $4.45 on my paypal account, instead of being refunded $27.99
This was about a week ago, and since then I have been requesting a call from ebay customer support every day. They never call. I tried calling them once and was on hold for about 2 hours, I then had things to do and gave up.
Can someone please help me out? This is very frustrating.

Perhaps try calling paypal. A seller can't withdraw money from your account without your permission. If they did, you do indeed have a problem but it's with paypal and not ebay.
The two transactions may be unrelated.
If the ebay seller who was ordered to refund you did not have money in his/her paypal account (like an actual balance of funds) it would be withdrawn from their banking account or credit card. Those often take several days to cycle through the system. Your actual refund money may still be coming.

You've had two sales in the past month according to your feedback.
It is likely that the $4.45 is actually fees, either selling and shipping fees to eBay or processing fees to Paypal.
How is the $4.45 withdrawal actually described?
When you sell something you get charged about 10% on both selling and shipping (lower in some categories but always 10% on shipping) by eBay and 30 cents per transaction plus a percentage by Paypal .
Free Listings are only free to list not to sell. I hope this doesn't sound condescending, but a surprising number of new sellers don't know that.
You should also check your credit card, if you have one attached to your PP account. Refunds to buyers often go there, confusing the expectant buyer.

Go to your PayPal account and look at the details for each transaction.

I found the refund in my credit card history.
What thew me off was the message ebay had sent me:
"We issued you a full refund of US $27.99 on Jun 15, 2016. The refund includes the purchase price plus original shipping and has been credited to your PayPal account. Please log in to PayPal and view the History page if you don't see it in your PayPal balance."
hmmm... oh well it worked out in the end.

That is a truly dumb piece of misinformation on the part of eBay.
And all that would be needed would be : "If your Paypal account is backed by a credit card, the refund will go to the card rather than to your Paypal account."
The policy is actually helpful, since you are being charged interest on cc balances and reducing that balance is a Good Thing.
But sheesh.
